Here are few steps for this.
Note: Information is just as collected by me, it is not to blame process or finger point at any one if found in the message.
Procedure:

# Ration card is distributed in Ration card offices(Ahaar Ilakhe == Food Department) Please inquire with your neighbors for your area's ration card issuing office.
# You need to have the following documents..
1) Ration card transfer/deletion certificate from your older ration card(from native place)
2) Photo identification proof(voter's id, driving license)
3) Address proof(rental agreement or sale deed or electricity bill)
4) 4 pass port size photo graph of the person on whom the ration card needs to be taken( the same person should have the transfer cert in his/her name)
# Application form will cost you Re. 1/-
# they will charge ~rs.45 for the process. It is fairly straight forward and easy, but you may need to wait for some time.
# The application is in Kannada, so if you do not know to read and write kannada, you may need to take some help of your friends (or) In my case, I have seen the application issuing person herself is helping out in filling. I am not sure whether every one will do.
# Filled in applications will be accepted in the same office, with in 2-3 weeks of submitting the application, verification person will come and inspect home, he will do a check in kitchen and see how many Govt subsidized gas cylinders u have.
# After 4 weeks, you can go to the same office and get the verified form from and get the signature of the supervisor there and take photograph and get the new ration card then itself.
Note:
1. Keep all receipts till you get the ration card.
2. Ensure that while printing the ration card you have told about "need for new gas connection".
3. More than one person from a house cannot apply for ration card at the same time else they will hold others.
I know only this much hope it will help. I cannot answer any queries other than these steps.
